What is Form SS-5?
Form SS-5 is the single application for anything to do with a Social Security card: an original card for someone who's never had a number, a replacement for a lost or stolen card, or a corrected card after a legal name change through marriage, divorce or court order. There is never a fee.
You have to prove your identity, U.S. citizenship or lawful immigration status, and — for a name change — the event that changed it. The SSA only accepts original documents or copies certified by the issuing office, not photocopies or notarized copies. Many people can now start an original-card or name-change request online and only visit an office to show documents.
- Who needs it
- Anyone getting a Social Security number for the first time (including newborns, though hospitals usually handle that at birth), anyone replacing a lost card, and anyone whose legal name has changed and who needs the card to match.
- Deadline
- No deadline. Replace a lost card whenever you need it (you're limited to 3 replacements per year and 10 in a lifetime, with exceptions). After a name change, update your card before your name matters for work or taxes.
- How to submit
- Complete the form, then either bring it with your original documents to your local Social Security office, or mail it there — check the SSA's office locator. Some requests can be completed through a my Social Security account. The card arrives by mail, usually within about two weeks.
Form SS-5 — line-by-line
1–2 — Name
The name you want on the card, and (for a correction) the most recent name shown on your existing card.
3 — Prior names
Any other name you've used, such as a maiden name or a previous married name.
4–6 — Place and date of birth, citizenship
City/state or foreign country of birth, date of birth, and your U.S. citizenship or immigration status.
9–13 — Parents' information
Your mother's and father's names and Social Security numbers, if known (required for an original card for a child under 18).
14–15 — Today's date and daytime phone
So the SSA can contact you about the application.
16–17 — Mailing address and signature
Where to send the card, and your signature (a parent or guardian signs for a minor).

Does it cost anything to get a Social Security card?+
No. A Social Security card — original, replacement or corrected — is always free from the SSA. Any site charging a fee is a third party, not the government.
What documents do I need?+
Proof of identity (usually a U.S. driver's license, state ID or passport) and proof of citizenship or immigration status (a birth certificate or passport). For a name change, also the marriage certificate, divorce decree or court order. Originals or agency-certified copies only.
How long does the card take?+
Usually about 7–14 business days after the SSA has verified your documents.
Can my child get a number at birth?+
Yes — you can request it as part of the birth registration at the hospital, which is the easiest way. Otherwise use Form SS-5 with the child's birth certificate and proof of your identity.
